professionals can be listed on The Campaign's
web site with a donation of
$100 or more. If you have a web site, your donation
of $100 or more also entitles you to have a
link back to your web site.
To
be listed on the Our Supporters web page, simply
use the online payment method below or mail
your payment, payable to The Campaign, to our mailing
address.

Important
information for proper web site listing:
During
the online payment process, you will come to a web
page that contains a box titled "Special Instructions
and Comments." Please indicate how you want
your name or practice listed in this box or we will
assume you want it listed the same way it is listed
on your credit card billing information. For example,
if the name on your credit card is "John
Doe" and
you wish to be listed as "Dr. John Doe" or "John
Doe, M.D.," simply write that in the "Special
Instructions and Comments" box. If you have
a web site and you want your name to link to it,
please include the web site address.
If
for some reason you want your donation to remain
anonymous, just state that in the "Special
Instructions and Comments" box during the
payment process.
