Consumers
and others who join The Campaign as paid
members or make a donation of at least
$12 may choose to be listed on the Our
Supporters web page in the "Consumers
and Others" section.
Note:
To protect your privacy, you are not automatically
listed. You must request to be listed
after joining as a paid member or making
a donation.
"Others" who
may donate and receive a listing include
families, groups, clubs, organizations, etc. "Others"
may also include companies, health professionals
or retail stores that do not make a donation
sufficient to qualify to be listed in
corresponding sections of the Our Supporter
web page.
Gold
Members and those who make a donation
of at least $100 are eligible to have
a link back to their web site. To
be listed on the Our Supporters web page,
either Join The Campaign as a paid member
or make a donation using the online payment
method below. Or you may mail your donation,
payable to The Campaign, to our mailing
address.

Important
information for proper web site listing:
During
the online payment process, you will come to
a web page that contains a box titled "Special
Instructions and Comments." If you wish to be
listed on the web site, please indicate it in this
box. You can just say "please list" and
we will list the name that you fill in on your payment
information. If you wish to be listed
differently on our web site than your credit card
payment information, please let us know how you want
to be listed. For example, if the name on
your credit card is "John Doe" and
you wish to be listed as "The John Doe Family" or "John
and Mary Doe," simply write
that in the "Special Instructions and Comments" box.
Also,
if your donation is at least $100 and you want
your name linked to a web site, please list the
domain address in the same "Special Instructions
and Comments" box. For example, you could
write "Please list as John and Mary Doe
and link to the web site www.johndoefamily.com."
Want
to donate anonymously?
If
you want your donation to be anonymous
and you do not want to be listed at all,
don't fill in anything in the "Special
Instructions and Comments" box.
Not
listed, but should be?
If
you made a donation of $12 or more in
the past 12 months and are not listed
on the Our Supporters web page but want
to be, click
here to go to a form that will accommodate
that request.

a modification be made to a current listing.
